General Tips on Filling Forms
You will need to validate the form after finishing (button at the end of the form), it will make sure you did not forget anything and will create barcodes at the last page of the form. Do not include in your application a form that does not have barcodes.
You need to read instructions on the forms very carefully. Don't hesitate to proofread your filled forms, as you don't want to send an application with inaccurate information. This will either lead to the rejection of your application or to an increase in processing time.

Translation of Documents
You must add the following for any document that is not in English or French:
Translations made by a member of the family are refused. "Family members" may be a parent, grandparent, spouse, child, brother, sister, guardian, aunt, uncle, niece, nephew or first cousin.
An affidavit is a document in which the translator swears that the translation is faithful to the content of the original document, in the presence of an authorized commissioner in the country where the translator resides.
Translators who are certified members in good standing with one of the provincial or territorial translators or interpreters organizations in Canada are not required to provide an affidavit.
Abroad, translations are accepted if made by an authorized translator in good standing with a professional translation association. The translation include the translator's stamp or their member number. No need for affidavit in this case.

You need to provide the following supporting documents:
- Original of fee payment in an acceptable format.

- For you and your spouse or common-law partner: photocopies of your birth certificates, marriage , divorce, name change or death Licenses/Certificates.
- For your children, photocopies of birth certificates, adoption papers. If the other parent of your children is not accompanying you to Canada, you must submit a signed Separation Declaration for Minors Travelling to Canada: IMM 5604.
- Two photos meeting the requirements for you and each member of your family, accompanying or not. On the back of the one photo, write your name, date of birth, and date the photo was taken.
- Two self-addressed mailing labels, (i) one in English or French and, (ii) one in the official language of your country of residence.
- Photocopy of your current immigration status if your country of residence differs from the country of citizenship listed on your passport.
- Original of police certificates from each country or territory where you have lived for six or more months in a row since the age of 18. You can learn here how to get the right police certificate.
Self-Employed Persons
You need to fill and sign the following forms:

Parents and Grandparents Sponsorship
You need to fill and sign the following forms:

- Application to Sponsor, Sponsorship Agreement and Undertaking: IMM1344. This form needs to be completed and signed by the sponsor, the co-signer if applicable, and the person being sponsored.
- Financial Evaluation for Parents and Grandparents Sponsorship: IMM5768. This form has to be completed and signed by the sponsor, and the co-signer if applicable.
- Income Sources for the Sponsorship of Parents and Grandparents: IMM5748. This form needs to be completed by the sponsor and/or the co-signer if the sponsor answered "No" to question 8 and/or if the co-signer answered "No" to question 16 of the form IMM5768 above.
- Statutory Declaration of Common-law Union form if you are in a Common-law Union: IMM 5409.
- General Application Form for Canada: IMM0008.
- Additional Dependents/Declaration: IMM0008DEP, if applicable.

- Canadian Citizenship Certificate or card (both sides), or
- Canadian birth certificate (if you were born in the province of Quebec, only a birth certificate issue by the Directeur de l'état civil is acceptable), or
- Canadian citizenship certificate without photo issued before February 15, 1977, or
- Canadian passport (pages showing passport number, date of issue and expiration, photo, name, surname, place and date of birth of holder).
- Photocopy of your marriage certificate, if you have a co-signer and he or she is your spouse (if you were married in the province of Quebec, only a marriage certificate issue by the Directeur de l'état civil is acceptable).
- If your spouse or common-law partner is a co-signer, a photocopy of either a:
- Permanent Resident Card (both sides) or, if you were not issued a Permanent Resident Card, your Record of Landing (IMM 1000) or your Confirmation of Permanent Residence (IMM 5292 or IMM 5688) or
- Canadian Citizenship Certificate or card (both sides), or
- Canadian birth certificate (if you were born in the province of Quebec, only a birth certificate issue by the Directeur de l'état civil is acceptable), or
- Canadian citizenship certificate without photo issued before February 15, 1977, or
- Canadian passport (pages showing passport number, date of issue and expiration, photo, name, surname, place and date of birth of holder).
- Proof of the parent or grandparent's relationship to the sponsor:
- Photocopy of the sponsor's birth certificate (If you were born in the province of Quebec, only a birth certificate issue by the Directeur de l'état civil is acceptable).
- If you are sponsoring your grandparents, you must also include a photocopy of your mother or father's birth certificate.
- Copy of travel document or passport for the principal applicant and each accompanying dependent.
- Proof of principal applicant's relationship to an accompanying dependent spouse or common-law partner;
- Photocopy of marriage certificate, OR
- Statutory Declaration of Common-law Union (IMM 5409).
- Proof of principal applicant's relationship to accompanying dependent children: photocopy of birth certificate for each accompanying dependent child.
- Two photos meeting the requirements for you and each member of your family, accompanying or not. On the back of the one photo, write your name, date of birth, and date the photo was taken.
Adopted Children and Other Relatives Sponsorship
You need to fill and sign the following forms:

- Application to Sponsor, Sponsorship Agreement and Undertaking: IMM1344. This form needs to be completed and signed by the sponsor, the co-signer if applicable, and the person being sponsored.

- Medical Condition Statement: Here. This need to be filled out if sponsoring a child you intend to adopt, or an orphaned brother, sister, nephew or niece.

- If you had previous marriages or common-law relationships, photocopy of the following documents:
- Proof that you are separated (Income Tax forms, legal documents), if you are separated.
- Divorce certificate, if you are divorced.
- Annulment certificate, if the marriage was annulled.
- Death certificate, if your former spouse or common-law partner is deceased.
- Declaration of severance of common-law relationship.
- Photocopy of the your birth certificate if sponsoring your mother or father (if you were born in the province of Quebec, only a birth certificate issue by the Directeur de l'état civil is acceptable).
- Photocopy of the adoption order, if you are sponsoring an adopted child.

Work in Canada - Temporary Residence - Forms and Supporting Documents
Work Permit

If you apply through a Visa Application Center, you will need to fill a waiver and consent forms that you can find in your country's Vfs.Global website (each country has its own waiver and consent forms).

- Application for Work Permit made Outside of Canada: IMM1295.
- Family Information form for every person 18 years or older: IMM5645 or IMM5707 depending on your region.
- Statutory Declaration of Common-law Union form if you are in a Common-law Union: IMM 5409.
- Use of a Representative form if you are using the services of a representative: IMM5476.
- Document checklist: IMM5488.
You need to provide the following supporting documents:
- Fee payment in an acceptable format. Please ensure to include the application processing fee and, if applicable, the biometric fee. If you apply on paper at a Visa Application Center, you can pay the $155 Work Permit fee (+$85 if biometrics) there. If you are applying for an Open Work Permit, you will have to add a $100 fee (so a total of $255). You can check your country's means of payment here.
- Photocopy of the information page(s) of your valid passport or travel document which includes: passport number, issuance and expiry dates, photo, name, date and place of birth.
- Two photos meeting the requirements. On the back of the two photos, write your name and date of birth. If you need your biometrics to be taken, you will no need to bring the two photos.
- Proof indicating you meet the requirements of the job being offered (diplomas, degrees or certificates, experience letter from past and/or current employers, etc.).
- A copy of the Labour Market Impact Assessment (LMIA) provided by Employment and Social Development Canada (ESDC) to your employer, and a copy of your job offer from your prospective employer, OR an Offer of Employment to a Foreign National Exempt from a Labour Market Impact Assessment (LMIA) (either an [IMM5802] or the Offer of Employment ID number).
- If you are a Provincial Nominee, a copy of your nomination letter and a R204c) letter issued by the province or territory.
- "Certificat d'acceptation du Québec" (CAQ) if planning to work in Quebec.
- Photocopy of your Marriage License/Certificate if married.
- Photocopy of your current immigration status if your country of residence differs from the country of citizenship listed on your passport.
- You medical exam report, if applicable.

Bridging Open Work Permit (BOWP)

- Two photos meeting the requirements. On the back of the two photos, write your name and date of birth. If you need your biometrics to be taken, you will no need to bring the two photos.
- Letter of acceptance from where you intend to study, it must be issued by the Canadian institution on official letterhead, showing the exact amount of tuition fees you are required to pay, the anticipated starting and finishing dates, and the date by which you need to register.
- "Certificat d'acceptation du Québec" (CAQ) if enrolled in an institution in Quebec.
- Proof of financial support. Examples of supporting documents are provided here.
- If the duration of your studies in Canada will be less than a year, proof of financial support for the duration of your studies in Canada.
- If the duration of your studies in Canada will be more than a year, proof of financial support for the first year of your studies in Canada.
- Photocopy of your Marriage License/Certificate if married.
- Photocopy of your current immigration status if your country of residence differs from the country of citizenship listed on your passport.
- Your medical exam report, if applicable.

Post-Graduation Work Permit (PGWP) and Co-op/Internship Work Permit

- Use of a Representative form if you are using the services of a representative: IMM5476.

You need to provide the following supporting documents:
- Fee payment in an acceptable format for the PGWP only.
- Photocopy of the information page(s) of your valid passport or travel document which includes: passport number, issuance and expiry dates, photo, name, date and place of birth, stamps made by the Canadian authorities, and any other marked pages.
- Photocopy of your current immigration document.
- For the PGWP only: a document from your institution stating you completed your program of study: (i) your final transcript, or (ii) a letter from your institution, and/or formal notice of graduation.
- For the PGWP and Quebec graduates of vocational and professional training programs only: an official letter from the school that confirms the length of the study program (in hours or months) and the program code.
- For the Co-op/Internship Work Permit only: a letter from your school that confirms all students in your program need to complete work placements to get their degree.
Extend your Study Permit
You need to fill and sign the following forms:

You need to provide the following supporting documents:

- Photocopy of the information page(s) of your valid passport or travel document which includes: passport number, issuance and expiry dates, photo, name, date and place of birth, stamps made by the Canadian authorities, and any other marked page.
- Letter from institutional registrar and/or photocopies of transcripts from your last two periods of study.
- "Certificat d'acceptation du Québec" (CAQ) if enrolled in an institution in Quebec.

- If the duration of your studies in Canada will be less than a year, proof of financial support for the duration of your studies in Canada.
- If the duration of your studies in Canada will be more than a year, proof of financial support for the first year of your studies in Canada.
- Photocopy of your Marriage License/Certificate if married.
- Photocopy of your current immigration status.
"Certificat d'Acceptation du Québec"
If you apply on paper, you will have to fill the Application for a Québec Acceptance Certificate for studies form. When applying online, you will have to answer the same questions that are in the form.
When applying online or on paper, you will have to provide supporting documents. Below is the list of documents required:
If you live or reside in Austria, Canada, United States, France (including La Réunion), Greenland, Hong Kong, Mexico, Monaco, or Saint-Pierre-et-Miquelon, you will need to also provide (i) proof of your financial capacity, or (ii) a recent proof of financial capacity of the person who supports you financially and a Declaration of financial support signed by this person (forms here). Depending on your situation, you have to provide originals or photocopies of the all or part of the supporting documents listed here.
You need to demonstrate that you have the financial means to cover the following expenses for the duration of your stay:
If a child under 17 is applying, and accompanied by one parent, they will need to also provide:

The documents provided must be in French or English or be accompanied by a translation into French or English, signed by a certified translator.
When applying online or on paper, you will have to provide supporting documents. Below is the list of documents required:
- Original of a recent passport-size identity photo (35 mm x 45 mm), with your name on the back,
- Photocopy of the page(s) of your passport with personal information and photo,
- Letter of admission from the registrar (or Admissions Office). If a child under 17 is accompanying his/her parent who holds a study or work permit, he will not need this document.
If you live or reside in Austria, Canada, United States, France (including La Réunion), Greenland, Hong Kong, Mexico, Monaco, or Saint-Pierre-et-Miquelon, you will need to also provide (i) proof of your financial capacity, or (ii) a recent proof of financial capacity of the person who supports you financially and a Declaration of financial support signed by this person (forms here). Depending on your situation, you have to provide originals or photocopies of the all or part of the supporting documents listed here.
You need to demonstrate that you have the financial means to cover the following expenses for the duration of your stay:
- tuition fees,
- transportation expenses (return ticket to your country of origin),
- settling-in expenses in the first year ($500),
- annual living expenses (minimum of CAN $12,538 for one adult, $6,270 for one child, $18,388 for two adults, $16,849 for one adult and one child, $20,598 for two adults and one child, $22,231 for two adults and two children), and
- cost of medical and hospitalization insurance for yourself and accompanying family members (about $600-1,000/year for a single person). If you are a citizen from a set of countries that signed reciprocity healthcare agreements with Quebec, you won't need to demonstrate financial means for this point. Find out here if you are a citizen of those countries.
If a child under 17 is applying, and accompanied by one parent, they will need to also provide:
- If the parent accompanying the child has sole custody, a photocopy of an official document attesting the latter.
- If the parent accompanying the child has not sole custody:
- an Original of a Declaration of consent (forms here) to the child’s stay in Quebec, signed by the parent who is not accompanying the child,
- a photocopy of the child’s birth certificate with the name of his parents, and
- a photocopy of an identity document of the non-accompanying parent bearing his or her signature.
If a child under 17 is applying, and not accompanied by any parent, they will need to also provide:
- a photocopy of the child’s birth certificate with the name of its parents,
- a photocopy of an identity document of each parent with their signature,
- the Original of the Declaration of parental authority (forms here) by a resident of Québec who is a Canadian citizen or permanent resident and at least 18 years old. This declaration must be signed by both parents or by the legal guardian. If only one parent has signed, a photocopy of an official document attesting that the signing parent has sole custody of the child,
- the original of the Declaration of guardianship (forms here) for a child under age 17 for the full duration of the CAQ or until the child is 17 years old, signed by the child’s guardian in Québec, and
- a photocopy of a document showing that the child’s guardian in Québec is a Canadian citizen or permanent resident.
The documents provided must be in French or English or be accompanied by a translation into French or English, signed by a certified translator.

You need to fill and sign the following forms:

If you apply through a Visa Application Center, you will need to fill a waiver and consent forms that you can find in your country's Vfs.Global website (each country has its own waiver and consent forms).

- Application for Temporary Resident Visa: IMM5257.
- Family Information form for every person 18 years or older: IMM5645 or IMM5707 depending on your region.
- Statutory Declaration of Common-law Union form if you are in a Common-law Union: IMM 5409.
- Use of a Representative form if you are using the services of a representative: IMM5476.
You need to provide the following supporting documents:
- Fee payment in an acceptable format. Please ensure to include the application processing fee and, if applicable, the biometric fee. If you apply on paper at a Visa Application Center, you can pay the $100 fee there. You can check your country's means of payment here.
- Photocopy of the information page(s) of your valid passport or travel document which includes: passport number, issuance and expiry dates, photo, name, date and place of birth.
- Two photos meeting the requirements. On the back of the two photos, write your name and date of birth. If you need your biometrics to be taken, you will no need to bring the two photos.
- Proof of financial support. Examples of supporting documents are provided here.
- Photocopy of your Marriage License/Certificate if married.
- Purpose of travel. Letter of Invitation for example.
- Photocopy of your current immigration status if your country of residence differs from the country of citizenship listed on your passport.
- Minors travelling alone or with one parent must provide custody documents or a letter of authorization from the other non accompanying parent or a letter of authorization signed by both parents or legal guardians.
